Steps for Participation in the Goodie Bag Program

1. Download:
   1. Goodie Bag Form (PDF*)
      *requires PDF viewer
   2. Avery Label Template (MS Word)
2. Call: Matt Nolin to participate in the goodie bag program. (You can not sign up for the goodie bag program through Elite Racing -- you must contact "The Sample Guy" to sign up)
3. Payment: Arrange to make payment with The Sample Guy.
4. Shipping: You will receive a shipment authorization code and the shipping address when you sign up for the goodie bag program.
5. Deadline: Inserts must be shipped to "The Sample Guy" goodie bag stuffing warehouse in time to arrive between August 18 and August 29, 2008.
